Symptom                  Possible Cause      Corrective Action
Unit will not operate           No power to unit        Verify power supply is correct                                     
                                                                                   and present at actuator. Verify 
                                                                                   power leads are connected in
                                                                                   accordance with the applicable
                                                                                   wiring diagram. Inspect for blown                             
                                                                                   fuse, tripped circuit breaker or                     
                                                                                   overload or open disconnect switch.

                                    Loose or incorrect Wiring    Check wiring for proper connection                 
                                                                                    in accordance with the applicable 
                                                                                    wiring diagram.Check also for tight 
                                                                                    connections.
                                    Limit switches not set or
                                    incorrectly set                        Check limit switch settings for 
                                                                                    agreement with the applicable
                                                                                    wiring diagram. If not set, then the                         
                                                                                    switches in accordance with the 
                                                                                    appropriate instruction manual.

                                    Torque switch open               Visually inspect torque switch to
                                                                                    verify that the contacts are closed. If
                                                                                    contacts are open, further the 
                                                                                    investigation is required to 
                                                                                    determine the cause. In this case, the 
                                                                                    unit will visually have been
                                                                                    operating satisfactorily previously.

                                    Foreign material on               Inspect and clean all switch
                                    contacts preventing good      contacts in accordance with the
                                    electrical contact                    appropriate instruction manual.
                                                                                    If foreign residue is from corrosion
                                                                                    due to moisture, the installation 
                                                                                    should be inspected for proper                     
                                                                                    integrity.

Motor runs but no         Actuator not coupled        If actuator coupled to manual gear-

                                    Worm gear worn out            Disassemble and inspect worm gear

                                   Clutch trippers misaligned    Inspect trippers to see if they are 
                                    (SMB-000/00)                      stuck behind the tripper arm, 
                                                                                   keeping the unit in handwheel mode.
                                                                                   Re-adjust trippers if necessary.

                                   Splined adapter installed        Inspect and verify splined adapter
                                   upside down (HBC only)       installed as shown in HBC.

Unit operates w/o    Shock or vibration causing    Review installation for excessive

Unit operation is      Damaged gearing                 Disassemble unit in accordance with

                                                                                and inspect gearing for rough or 
                                                                                damaged spots. Replace the part.

                                    Inadequate lubrication       Inspect quality and quantity of lube in
                                    (poor quality/not enough)  accordance with the appropriate
                                                                                instruction manual. Add or replace the
                                                                                lube as required.

                                    Gearing misaligned            Inspect gearing for uneven wear
                                                                                pattern. Most often seen in a motor
                                                                                gear set.

                                    Worn or damaged              Disassemble unit, refurbish/replace
                                    bearings                              all bearings, seals and lubricant.
                                                                                Inspect motor bearings and refurbish
                                                                                as required.

Fuse Blown                Incorrect fuse size             Verify fuses are sized correctly. 
                                                                                Consult ASI if guidance is needed.

                                    Pinched wire                      Inspect control compartment to
                                                                                ensure the cover isn’t pinching or
                                                                                making contact with the wires.

                                     Power Surges                    Investigate control circuit for surges.

Premature torque       Torque switch setting        Increase torque switch setting as 
switch trip                   too low                                required up to but not exceeding
                                                                                maximum setting established by
                                                                                torque switch limiter plate.

                                    Valve stem  or stem nut    Inspect valve stem for bronze flakes or
                                                                               evidence of bronze tracked on stem.
                                                                               Replace stem nut. (also see inadequate
                                                                                lubrication above)
 
                                    Valve packing too tight    Inspect valve packing for excessive
                                                                               tightness. Repair valve by replacing
                                                                               packing as required.

                                    Actuator housing cover    Verify housing cover installed and
                                    incorrectly installed          shimmed as outlined in the appropriate
                                                                              instruction manual. 

                                   Torque switch not            Inspect torque switch for proper
                                    balanced                           balance and adjust if required.

                                    Bent valve stem               Inspect valve stem for damage. Usually
                                                                             caused by excessive force applied to 
                                                                             valve stem. Replace valve stem and
                                                                             inspect valve for any other damage.

Torque switch fails    Output rotation not in    Consult with factory or ASI to verify
to stop actuator          agreement with unit        design rotation. If actual rotation

                                    Incorrect installation        Verify torque switch was installed with

                                   Torque switch not              Inspect torque switch for proper
                                    balanced                             balance and adjust it if required.

Torque switch            Non-locking gear set        Consult with factory to determine if
remakes after                                                        gear set is non-locking and to verify
torque out                                                              application.

                                    Excessive worm-gear        Disassemble and inspect worm gear.
                                    wear resulting in too          Replace if required.
                                    much backlash

Reversing starter        Starter undersized            Verify starter is sized correctly for
failure                                                                     application. Consult ASI to evaluate.

                                     Excessive cycling            Review application to determine
                                                                              number of starts per hour and review
                                                                              with factory if more than a 100.

Excessive current        Valve running loads        Inspect valve for possible causes of
draw                             higher than expected       high running load. In particular check
                                                                               for excessive tightness of valve packing
                                                                               and for proper lubrication of valve
                                                                               stem. Consult ASI for review of                                     
                                                                               application.

                                    Actuator housing cover    Verify housing cover is installed and
                                    incorrectly installed          shimmed as outlined in the appropriate
                                                                               instruction manual.

Motor overload        Excessive current draw    See Above.

                                  Incorrect overloads           Verify proper overloads selected for
                                                                              motor rating.

Excessive gear-wear    Excessive loads              Verify actual loads are in accordance
                                                                              with both start and run capabilities of
                                                                              the appropriate unit.

                                    Inadequate lubrication    Inspect quality and quantity of lube in
                                                                              accordance with both start and run
                                                                              capabilities of the appropriate unit.

                                    Improper alignment        Inspect worm gears for evidence of
                                                                             uneven wear pattern. Consult ASI.
